Clinical Application of the Levothyroxine Absorption Test in the Diagnosis of Pseudo-Malabsorption

چکیده
We present a variation and clinical application of the Levothyroxine Absorption Test (LAT) in the identification of pseudo-malabsorption of levothyroxine in uncontrolled hypothyroidism, for which there is no current gold standard protocol, and a variety of methods proposed in the literature. The LAT was conducted over 5days with thyroid function tests conducted pre-LAT, then at day 1 and 5. The thyroid stimulating hormone levels became completely suppressed indicating that there had been previous non-compliance with medication.

Rapid Levothyroxine Absorption Testing: A Case Series of Nonadherent Patients
BACKGROUND Nonadherence to levothyroxine therapy is one cause of persistent hypothyroidism. To distinguish nonadherence from malabsorption, a levothyroxine absorption test is required. Typically, this test measures the serum free thyroxine (FT4) response to 1000 mcg of oral levothyroxine over 4 to 24 hours. Published data indicate that serum levels of FT4 are at or near their peak 2 hours after...

متن کاملThe influence of lactose intolerance and other gastro-intestinal tract disorders on L-thyroxine absorption.
The preferred treatment for hypothyroidism is oral levothyroxine (LT4) ingestion, in doses that ensure a sustained state of hormonal balance. Many different factors may significantly influence the absorption of LT4, including: interval between the ingestion of the drug and the last meal, eating habits, and different functional and organic pathologies of the gastro-intestinal tract. The main pur...
متن کاملLevothyroxine Tablet Malabsorption Associated with Gastroparesis Corrected with Gelatin Capsule Formulation
Treatment of hypothyroidism with levothyroxine sodium often requires multiple dose adjustments and can be complicated by patients with gastric and intestinal dysfunction that limits absorption. In these cases, doses are often titrated higher than commonly used in clinical practice. Multiple formulations of levothyroxine are currently available and some may be preferred in cases of malabsorption...
